How to fill it out by hand

Work line by line on the schedule of values first: enter each item's scheduled value, what you billed on previous applications, this period's work, and any stored materials — then total the columns. Carry the totals to the cover page, apply your retainage rates, subtract previous applications, and the remainder is the payment due. Double-check that the schedule of values sums to the contract sum; mismatched totals are among the most common reasons applications bounce.

Is the template free?

Can I print it and fill it out by hand?
That's exactly what it's for. The template is a letter-size PDF with ruled entry lines sized for handwriting — value columns on the schedule of values, summary lines on the cover, and signature and notary blocks.
Does a pay application need to be notarized?
Only if your contract or the payer requires it. The template includes an optional notary block you can toggle on or off before downloading.
Is there an Excel version?
Yes — the same template as an .xlsx workbook: a G702-style cover and a continuation sheet with as many rows as you choose, with the totals, retainage, and payment-due formulas already wired so the workbook calculates as you type. It opens in Excel, Google Sheets, and LibreOffice.
How many line items does it support?
Up to 20 schedule-of-values lines — a practical limit for hand-written math. If your project needs more, the free generator supports up to 60 lines and calculates every total for you.
Is this an official AIA G702/G703 form?
No. It's an original layout that captures the same information those forms are used for — schedule of values, retainage, and payment due. If your contract strictly requires official AIA documents, purchase them from AIA Contract Documents.
